More people these days are choosing to be cared for in their own homes rather than in long term care centers. There are many types of home care services to support individuals who have different needs for ongoing help, short term help, or help from time-to-time. Arranging home care can be challenging. We can discuss all of your needs and your concerns in the PRIVACY of your home. Knowing where to start, and what type of care or services are needed can be a daunting task, with your help we will prepare a care plan with services tailored to your wants and needs. Look at the services we provide, under the services tab and see which best suits you. If you need to combine services we can arrange it.
There is a minimum of 1- 2 hours for all services provided, See Services /Price Tab to see minimum hours per service and a price range for the services provided. There is no maximum number of hours per services except homemaker, we have a maximum of three hours per day as we are not a cleaning service. We can discuss the amount of time actually needed for the services you require. We are easy to talk to, and down to earth, our purpose is to make the whole experience of searching for quality home care a pleasurable one. We speak in terms you understand and are readily available to provide further assistance even after our free initial consultation/assessment. We are currently servicing Fairfield County, additional areas will be added as we grow.
Soni HomeCare is a new and independant high-quality home care agency that provides home care services to those who wish to remain living independently at home. A variety of person-centered care services are available from companion care, to live in care, and personal care assistance; there is a care package suitable for all.
Caregivers are carefully selected for their compassion and care experience, all employees are vetted thoroughly.

We offer a range of home care services (see our services below), medication reminders, light housekeeping, companionship, and more. Our services are tailored to meet the needs of each individual, and we work closely with families and caregivers to ensure that our clients receive the best possible care.
Preparing meals and feeding
Adequate nutrition and hydration are a key well-being component for the care recipient, our caregivers can assist in meal planning, preparation and feeding, preparing a simple breakfast or sandwich for lunch. Where possible we will work with the individual in preparing a meal; with the individual managing part of the task, so they remain involved. Our caregivers are capable of preparing a meal from ingredients and instructions provided when an individual asks, but for many the ease and speed of micro-waved meals are sufficient to provide a hot and healthy meal.
Assistance in getting dressed and undressed
Arthritis, stroke, general frailty or short-term recovery from falls and accidents mean that we can be required to assist in getting an individual dressed or undressed. As with other personal care tasks the care workers are trained to work patiently with safety of the individual while getting dressed is the prime concern.
Medication Reminders
One of the more important services we offer is medications reminders. Medications can be an issue for the elderly with frequent changes in content and when they’re to be taken.
As a part of our care visits and with the express consent of the individual receiving services, our caregivers will give the appropriate medication reminder at the time medications should be taken during the visit. Any refusal or failure to take medication, will be noted and appropriate agency/individual will be notified.
Shopping, laundry and domestic tasks
The difficulty of managing shopping trips can be a problem for some elderly and those with mobility and transport issues. Our caregivers will help write a shopping list accompany you to the shops, or go on their own, return with exact change and receipt, and assist with putting groceries away.
Other domestic tasks that can become a problem for older adults such as wet laundry and light domestic tasks around the house including cleaning. All of these activities can be included as a part of a care package.
Bathing, showering and toiletry needs
Taking a bath or shower on your own requires significant cognitive and physical abilities of flexibility, balance strength and confidence. If you doubt this try writing a step-by-step list of all the things you have to do when you next have a bath! You may find you do actually need assistance. Many elderly people choose not to have a bath because of their fear of falling or the fear of being embarrassed while being stuck in a bath or having someone assist them for the first time in many years. We understand all those fears and the additional concerns about being left shivering in the cold, with wet hair after they get out. A well-managed bath can be relaxing as well as a functional necessity for general wellbeing.
We will produce a personal method statement for managing the bathing or showering process covering the obvious safety aspects through to personal preferences e.g. hair washed first or last etc. Our caregivers are trained to follow this part of the care plan to give a safe and relaxed experience. Support in maintaining some form of normality around toileting and toiletry needs is as important to your loved one as it is to you. The care plan will include a regime to manage the hygiene aspects as well as safe disposal designed in a way to protect an individual’s dignity and self-esteem.
Personal Care
The requirement for assistance in getting in or out of bed and the personal tasks to get ready to face the day or retire for the night is a common requirement for many elderly people at home. It is one of the most common times when falls and accidents occur. Our care workers are trained in assisting and managing personal care tasks for the elderly. We are patient and attentive over tasks such as transferring safely from bed to bathroom, cleaning teeth or dentures, washing the face and other body parts in a manner that is sensitive to the intimacy of the situation. We are always conscious of the need to maintain our customer's dignity and work in a way to assist our customer with the tasks they cannot do rather than take over the task completely. Our professionalism ensures that the care recipient gets appropriate home care.

Will I have the same caregiver(s)?
YES! SHC aims to always have the same caregiver providing the home care services, so that you get to know each other well. Of course, caregivers need days off and sometimes may be unwell. At such times another caregiver will be matched as closely as possible to your needs. Whenever possible your care coordinator who you will already know well and have a direct line to, will contact you personally to inform you of any changes in caregiver. This way you receive continuity of your care.
How will you and your staff respect my privacy and dignity?
We take the responsibility of dealing with personal and sensitive information very seriously and will never disclose data without your express permission (hipaa). Our Caregivers have been put through up to date and relevant training to ensure your needs are met to the highest standard and all of them understand their role and how to support your privacy and dignity.
What if I find I just don’t “fit” with my caregiver(s)?
Matching the right caregiver(s) to you is very important at SHC. During our rigorous recruitment process, induction and training, we get to know our caregivers well, so we can find you the right person. However, we never expect you to make do and if the match doesn’t work for any reason, we will work with you to find you a caregiver that you feel more comfortable with. You will have a direct line to your care coordinator, whose job it is to ensure you are content and that your needs and wishes are being listened to.
What if I need to speak to someone out of office hours?
We are here for 24hrs a day, every day! Come rain or shine. We have an out of hours emergency call service. However, for non-urgent calls we would politely request that you contact us during the hours of 8:30am - 4:30pm, Monday to Friday.
What are the charges and how do I pay them?
We fully endorse transparency and clarity of pricing. So, there are no hidden charges, costs or extras to surprise and worry you. We include services offered and variable rates charged. Prices for each service are clearly laid out.
Prices are for private pay individuals as well as insured individuals. Payment can be made by cash, check or money orders.

To appropriately provide services to, we must obtain information from you including your full name and address, we will use and disclose this information and other information we collect in the ways described below. These individuals and facilities need your information to provide care, and to coordinate and provide services (such as prescriptions, lab tests, meals, and x-rays).

We use your information to create a bill and disclose your information when we send the bill to your insurance company, you, or a third party. We may also disclose your health information to outside entities without your consent or authorization in the following circumstances:
· Required by Law. We disclose information as required by law. For example, we are required to report gunshot wounds to the police. We are also required to provide information to the Secretary of the Department of Health and Human Services to demonstrate our compliance with HIPAA.
· Public Health Purposes. We disclose information to health agencies as required by law for preventing or controlling disease. Examples are reporting of sexually transmitted, communicable, and infectious diseases.

· Individuals Involved in Care. We may tell your friends, relatives and other caretakers’ information which is relevant to their involvement in your care.
· Disaster Relief. We may disclose information about you to public or private agencies for disaster relief purposes.
Except as provided above, we will obtain your written authorization prior to disclosure of your information for any other purpose. Specifically, written authorization is required prior to the disclosure of your information:
· Marketing. We will not use or disclose your information for marketing purposes, other than face-to-face communications with you or promotional gifts of nominal value, without your written authorization.
· Sale of Information. We will not sell your PHI without your written authorization, including notification of the payment we will receive.
Where a disclosure is made under your written authorization, you have the right to revoke the authorization at any time. Revocation of an authorization must be in writing. The revocation is effective as of the date you provide it to Soni HomeCare and does not affect any prior disclosures made under the authorization.
If a state or federal law provides additional restrictions or protections to your information, we will comply with the most stringent requirement.
Your Rights
· You have the right to request a restriction on how information about you is used and disclosed. If you want to request a restriction of a use or disclosure of your information, contact our Privacy Officer at the number listed at the beginning of this form. We are required to agree to a request for a restriction related to disclosure of information to your health plan for payment or healthcare operations where you pay for the service in full. We are not otherwise required to agree to any restriction on the use or disclosure of your information.
· You have the right to request communications with you be made at an alternative address or phone number. To request that communication be made at a different address or phone number contact our Privacy Officer at the number listed at the beginning of this form.
· You have the right to inspect and copy your medical record. To inspect and copy your medical record a request must be made in writing. To obtain a form contact our Privacy Officer at the number listed at the beginning of this form.
· If you believe the information, we have about you is incorrect or incomplete you may request that we amend your medical information. Your request must be made in writing. To request an amendment of your information, contact our Privacy Officer at the number listed at the beginning of this form.
· You have the right to receive an accounting of disclosures, a list of individuals and entities that received your health information in the last six (6) years for reasons other than treatment, payment, or healthcare operations and other certain disclosures. You may receive one (1) free accounting during a twelve (12) month period. If you request more than one (1) accounting in a twelve (12) month period, you will be charged a fee.
· You have the right to request a paper copy of this Notice. Our Duty
· We are required to follow the terms of the current Notice.
· We may change the terms of this Notice and the revised Notice will apply to all health information in our possession. If we revise this Notice, a copy of the revised Notice will be posted, and a copy may be requested from our Office Staff at the number listed at the beginning of this form.
